I cant help but like the pricing page for GetFeedback.com
Not so sure I like it from a customer perspective, but boy it's hard not to consider a 50% discount for paying 2 years up front. That said, it's a little hard to justify so few feedback interactions per month costing so much IMO.
Still a super example of x- and up-sell via a pricing table.
